The doctors welcome new patients who live within our practice area.
Who will be your doctor?
Your registration will be with the practice - Lime Tree Surgery. You will be given a named doctor responsible for care however you can see any of the doctors practising at the surgery. Although you may have to wait longer to see a specific doctor, for continuity of care, we would encourage you to consult the same doctor as far as possible.
Your First Appointment
When you register with the practice you will be asked to complete a health questionnaire (one for each member of your household). This can be collected from our reception desk or downloaded below, printed and filled out to save you time. If you wish, you can make an appointment for a new patient check.
Summary Care Record
When registering with the practice you will also be asked whether you consent to your information being stored electronically and uploaded to a national database. This Summary Care Record is an electronic record which contains information about the medicines you take, allergies you suffer from and any bad reactions to medicines you have had.
Having this information stored in one place makes it easier for healthcare staff to treat you in an emergency wherever you are, or when your GP practice is closed. Lime Tree support the use of Summary Care Records and so if you do not complete the form below and hand it in, we will assume consent.

Patient Guide to GP Services
The practice welcomes new patients living within the boundary, which includes both of our sites (Lime Tree, Findon and Phoenix, Goring). The practice boundary line extends as far west as Titnore Lane, as far South as the shoreline and East to Sompting Road. (See the map below for a clearer representation of of our boundary lines).
Patients moving out of the practice area will routinely be asked to register at a nearer practice.
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. After three months you will need to register as a permanent patient.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered. To save time please download and fill out the temporary patient form before you arrive.

You are able to book appointments through our online services with a GP, request repeat prescriptions for any medications you take regularly and look at your GP record online. You are still able to use all other methods available for these services.

You will need to bring the completed form into the surgery along with identification, details of these requirements are on the registration form.
The practice has the right to remove online access to services for anyone that doesn’t use them responsibly.
This service is currently only available to those over 18 years of age.
