Accountable GP

Every patient registered at the surgery will have a named accountable GP, who are responsible for your overall care at the surgery. You will be informed of who your accountable GP is, when you first attend or contact the surgery after registration or alternatively you may wish to make an enquiry.

Where possible if a patient expresses a preference for an accountable named GP the surgery, will make reasonable efforts to accommodate this. All patients can still see any GP within the surgery.

All patients who are aged 75 and over, their named accountable GP will also work with relevant associated health and social care professionals to deliver a multi-disciplinary care package that meets the needs of the patient and ensure that these patients have access to a health check.
